Quick answer

You are eligible for Medicare at age 65 if you are a U.S. citizen or legal resident of at least five years. You also qualify at any age after 24 months of Social Security disability, or immediately with ALS or end-stage renal disease. Premium-free Part A requires 10 years of Medicare taxes.

Wondering if you qualify for Medicare? Eligibility comes down to your age, work history, and health. Here is how to know.

Who is eligible for Medicare?

Three groups qualify for Medicare. First, people age 65 and older who are U.S. citizens or lawful permanent residents who have lived in the country for at least five years. Second, people under 65 who have received Social Security Disability Insurance for 24 months. Third, people of any age with ALS (eligible the month benefits begin) or end-stage renal disease. Premium-free Part A depends on having 40 quarters of Medicare-taxed work, yours or a spouse's. If you are unsure where you stand, call 1-800-MEDIGAP for a free eligibility check.

What if I or my spouse did not work enough quarters?

If you have fewer than 40 quarters of Medicare-covered work, you can still get Part A, but you may pay a monthly premium, up to $518 in 2026, depending on your quarters. You can also qualify on a current, former, or deceased spouse's work record. Part B eligibility is not tied to work quarters, so you can enroll regardless, paying the standard premium. The rules can be confusing, so a licensed agent at 1-800-MEDIGAP can review your record and explain exactly what you will pay.

Frequently asked questions

What are the eligibility requirements for Medicare?+

You must be 65 or older, or under 65 with 24 months of Social Security disability, ALS, or end-stage renal disease. You also generally must be a U.S. citizen or a legal permanent resident who has lived in the country for at least five continuous years.

Can I get Medicare before 65?+

Yes. You can qualify before 65 if you have received Social Security Disability Insurance for 24 months, or immediately if you are diagnosed with ALS or end-stage renal disease. Otherwise, eligibility begins at 65. Call 1-800-MEDIGAP to confirm your situation.

Do I need to be a U.S. citizen to get Medicare?+

Not necessarily. Lawful permanent residents who have lived continuously in the United States for at least five years can also qualify, in addition to citizens. Meeting the work-history requirement for premium-free Part A is separate. A licensed agent at 1-800-MEDIGAP can clarify.

Does my income affect Medicare eligibility?+

No. Income does not determine whether you are eligible for Medicare; age, disability, and work history do. However, income can affect your premiums through IRMAA surcharges on Parts B and D for higher earners. Call 1-800-MEDIGAP to understand how income may affect your costs.

Am I eligible for Medicare if I never worked?+

Possibly. You can qualify for premium-free Part A on a spouse's work record if they have 40 quarters. If neither of you qualifies, you can still enroll by paying the Part A premium, and you can enroll in Part B regardless. Call 1-800-MEDIGAP for help.
